Step 1: Choose How to Apply or Renew

Your Medicaid benefits need to be renewed every year. Ohio will send you a notice when it's time to renew.

Step 2: Gather Your Documents

  • Proof of income
  • Proof of resources, like bank accounts, savings, or property
  • Proof of citizenship or immigration status
  • Information about any other health insurance you have
  • Details on any changes to your household or income

Step 3: Sign and Submit

Sign and date your form, then send it to your local County Job and Family Services office by mail, fax, or drop-off. If someone else is helping you apply, they can act as your authorized representative as long as you give written permission.
